There are four ways to begin. None is better than the others, and every route receives the same care from Mertis Williams, the Foundation's Memorial Steward.

Begin in the way that feels possible.

Route 01

Online form

Answer a short set of questions online. You do not need a Google account. Photographs may be sent separately by email or collected by Mertis in follow-up.

Email

If a form is the wrong shape for what you want to say, write about your child in your own words. There is no wrong length.

Paper

Download the approved paper form, complete it by hand, and return it by email or mail. Mertis can also send a paper copy on request.

Telephone

If writing it down is more than you can do right now, call the Foundation and ask for the Memorial Steward. We will write it with you.

What happens next

Your submission is not publication.

Mertis reviews the submission, follows up gently about anything needed, and prepares a private draft. Nothing appears on the Honor Wall until the family has seen the exact tribute and approved it in writing.

A family may request a correction or removal at any time and for any reason. There is no fee, and memorial placement is never connected to a donation.

Before you begin

Share what you can.

You do not have to answer everything at first contact.

Please include your name, your relationship to the child, and how to reach you. Then share the child's name, important dates if you wish, and the story you want the world to know. A photograph may be sent by email or arranged with Mertis.

Health information is optional. The Foundation may receive information for context without publishing it. Publication is decided only against the exact private draft the family reviews.

Does a submission publish automatically?

No. Mertis prepares a private draft, and nothing is published until the family approves the exact tribute in writing.

Is one submission route preferred?

No. The online form, email, paper, and telephone routes receive the same stewardship and review.

Can a family change or remove a tribute later?

Yes. A family may request a correction or removal at any time and for any reason.

Is there a fee or donation requirement?

No. There is no fee, and memorial placement is never tied to a donation.
